Low AHI & RDI - Still feeling terrible, not OSA?

Post by sleepy2448 » Thu Oct 12, 2017 6:00 pm

Hi everyone. Thanks so much for creating this incredible resource for folks with OSA. I was diagnosed with OSA (AHI of ~5/6) about 4 months ago. I have only really started to use my CPAP now. I have found over the last month I am not sleeping much better with the CPAP. I still wake up, have strangely low heart rate, brain fog and other symptoms.

I downloaded SleepyHead. Here is a readout for a "Typical" Night on the machine:

AHIApnea Hypopnea Index 3.40
Philips Respironics DreamStation
Auto CPAPModel 500X110 - J186765455272
PAP Mode: APAP (Variable)
Min 7 Max 20 (cmH2O)
Hours 10:17:58
Periodic Breathing 0.40%
Large Leak 0.00%
Clear Airway 0.10
Obstructive 0.39
Hypopnea 2.91
RERA 0.39
Flow Limitation 0.19
Vibratory Snore 0.00
Vibratory Snore (VS2) 0.19
Pressure Pulse 0.58
Breathing Not Detected 0.00%

I usually only have a couple of full obstructions (1-2) with many Hypopneas 10-30 per night. Do I need a higher pressure to shut these hypopneas down? or should I consider that my sleep problems lie elsewhere and I don't really have OSA? These numbers look like UARS to me...

I should also mention that I sometimes sleep at my girlfriend's house on a different mattress, I always sleep like a log right through the night and never have any issues. I don't even wear my CPAP there and I wake up buzzing with energy. Her mattress is more firm. Any ideas?

Thanks guys!

Do these numbers go along with the above stats where you showed 10 hours plus for having the machine on?
Did you actually sleep those 10 hours?
Do you take any medications of any kind? If so, what?
Do you find that you are waking often during the night for some reason or other?

And yes...a little more minimum pressure is usually what is done for hyponeas. Your numbers aren't horrible but combined with the RERAs and FLs it could point to less than optimal sleep quality. Usually hyponeas are easily dealt with. Probably 1 cm more minimum will do the job.

It's really hard to get a feel for what you machine is doing with just the stats. It's better if we can see the detailed report from the software.

Will reducing the hyponeas let you feel better? Don't know because sometimes the numbers aren't the whole story.
Worth trying though.

Hey xxyzx,

Thanks for the comments, where is that post located?

Sub 40bpm some nights. I have had bpm down to 28 before while sleeping.
