ASV is finally in my hands!

Post by RestlessinWoolley » Fri Apr 14, 2017 2:44 pm

Wonderful experience with then new DME, they are by far more professional then my old one. Due to a last minute decision change by my doctor solely due to to the algorithm and programming difference, I am the proud owner of the second DreamStation Bipap autoSV that the DME has setup..lol I was really hoping for the smaller Resmed unit and even the doctor wanted me on it, but she conferred with a colleague of hers and even he agreed the Respironics would work better for me. its identical in looks to my DS Auto Cpap but has a black face, and the menus are even back background.

I have a stern please do not monkey with her settings from my Doctor.

EPAP Min 6, Max 10, PS Min 6, MAx 12, BURR 10 (whats BURR? BAck up rate?) Press Max 22, BPM 10, Ti 1.2, Flex set on BiFLex, Flex Level 2 (the only setting she is okay with me changing) Ramp disabled, humidification Adaptive. After a few weeks and things clear up and my sleep gets better she will enable alarms..

We went thru about 5 different masks even a full full face, muy overkill, but settled on a ResMEd Quattro FX. it fit around my mouth better than the others, the AmaraView was a close second, but to keep it from leaking at higher pressure blasts it had to be too tight. And they set me up for 3 months into the future, the do it every 3 months not monthly to save on shipping costs, and even get extra filters if I decide to pick up at their local office.

Heated Hose as well as the Modem with the Oximeter Input requested in case she wants to have me on an O2 monitor if my centrals persist. So tonight will be an adventure for sure.

It sounds like you've got much better support from your medical team now. Good luck to you with the new machine!

Don't know what BURR is. Back up respiration rate maybe but then that's what I though this was "BPM 10" and I thought BPM was breaths per minute.
Should be covered in the manual though.
Looks like the apneaboard says they have the clinical manual for this very new Respironics model.
Request this
Philips-Respironics DreamStation AutoSV

from here

Instructions about 2/3 down the page.

I would have stuck with the Resmed Aircurve 10 ASV, but congrats on getting what you wanted.

What feature of the PR Dreamstation Auto SV Advanced was the game changer? The backup respiration rate IBURR) is kinda primitive compared to ASVauto on the Resmed, and sounds like a ST throwback.

I really wanted the ResMed unit, mostly due to its size and how much quieter it was. But Doc tossed in a last minute request as they felt the DreamStation ASVs Algorithm was a better fit for me, and had the prescription written as a drop in to a Respironics unit. My DME is amazing, so much clearer instructions, far more friendly, and no fighting to get what i wanted or needed. I was even more happy he let me try on a bunch of different masks before thinking maybe I would like the older Quattro FX. its just enough wider at the mouth it doesn't irritate my lips, tonight will be the test. He has one other mask I can try if I end up not liking it, but its special order for him, a gel mask, and totally clinical style. even the FX has the ports on the frame for o2 if needed.

Oh the big key was and I am totally guessing is I was on an old System One 60 Series ASV for the test, and I totally knocked out in minutes, and adjusted fast. That is my only thought to why, plus having the base Auto Cpap ive adjusted to the way it works. Ill be sure to ask at my compliance check in June. As she was not happy that I had the DS to start with.. the one thing I did not like on the AirSense 10 anyway was the pulse flutter...hated it.. But at least the Phillips unit really stepped it up for its top model, maybe see a software update for the others soon. Screen and be totally adjusted and is not blinding in the dark, the knob is trimmed in metal chromed, and feels better and more solid. the humidifier door opens to a full 90 on this one. my other one stopped at about 15 degrees short of 90 making it easy to spill water. The cord has a little hokey clamp to prvent accidental unplugging. oh and you have to hold the power button now to shut it off.. a failsafe.

So will see here in a few hours if that how I do, will do a SH dump in the morning..

Ill look into joining the forum, Pugsy the new DME included a Clinicians Booklet with the machine.. but like I said I can mess with anything BUT my pressures. she is going to monitor it remotely for a few weeks and make changes as needed, said not to get too comfy with one pressure as she might try a few things. And ill be back in the sleep lab in 6 months for one final night..

Respironics ASV have more customizable setting choices than the ResMed ASV.
You can tweak it for comfort a lot easier plus the Respironics ASV will actually flag centrals separately....ResMed ASV doesn't have a central apnea flag...it just has Apnea flag.

Respironics ASV is a little kinder/gentler in terms of its algorithm. ResMed is more aggressive with faster bigger changes in pressure and some people do better with one brand's way of doing its job than the other brand.
Minor pros and cons with either brand.

So far the only thing I am missing is seeing the actual temp for the hose, just gotta memorize what each number represents.. he had me at 86 on the 10, and set me on a 4 on the DS, plus being able to adjust humidity on the fly vs digging theu menus in the dark is a bonus to the Respironics Units

the two have absolutely nothing to do with each other.

asvauto is a varying EPAP, and has nothing to do with the breathing rate.

Well not able to do any SleepyHead dumps for now. It popped up that the BiBap ASV is not yet supported. Bummer

I was able to wear it a bit over 4 hours last night, I swore it was longer, but the machine wouldn't lie. I thought I had put it on and fallen asleep about 830-9 last night and woke up about 1230 took it off, as I was really plugged up due to a cold or my allergies, and I slept till 4, put it back on as it said I had not met my compliance for the night and slept till 615. Auto on and off is off, so I must of pulled it off earlier than I thought. its definitely going to take some getting used to it. the Quattro FX seemed to do its job and didn't leak. My 90% Pressure was 7.5 with an 1.3 AHI.

Compliance isn't your goal, it's what's required or they will reclaim your machine, and stop paying for your equipment. Use XPAP every time you sleep, period, anything else is a waste of your life! Jim

Oh totally, it was not a good night to start due to a head cold compounding things. I usually take it off for 15-20 minutes and back on but I ended up falling asleep a few more hours till I got a panic emergency phone call from a guy I do some work for. I wore my old CPAP for 117 nights only missing one night due to having my dog dying on my lap one night I was up all night, and the other for my sleep study for the ASV. Always 8-9 hours or more. it was just a huge step from a flat 6 cm to a 6 base and every slight inhale getting nailed with 12 or more.. lots more chances for leaks Ill be set for tonight for a full pull.. I can actually breathe today.

Restlessinwoolley. Maybe you can help me. I have a Resmed Airfit F10 full face mask. The height is just right, but it's too narrow and leaks at the corners of my mouth. Have you ever used a F10? How does the width of the F20 compare to the Mirage FX? Those are the two I'm looking at to replace my F10. Or possibly an Amara View. I need something a little wider.
