What is the official protocol for setting an AutoPap?
I hate to admit this, but I dont know what it is. Ive been running Autopaps since 2007 and "my own" way of doing it was how I did it. It was this: I set my minimum pressure as my in the sleep lab, titrated CPAP pressure of 10 as my minimum pressure. And most of the time, set my maximum as 20. In the last year or so, I sometimes set it back tighter to 14, as Ive found tightening the top pressure, that prevents abdominal irritation.
But whats the OFFICIAL protocol for setting up an AutoPap? Say, you know what your 95th percentile is (Resmed) or your 90th percentile (Phillips Respironics). Say your 95th is, 11.5.
On Resscan, 95th is 11.5. On Resscan, your thirty day average median is lower, its 7.3 or 8. And on Resscan, your maximum thirty day average pressure is, 14.
How should I set my APAP up on the consideration I find my 95th percentile too much to tolerate blowing all the time. Whats a realistic setup, but going by the industry official protocol?
I hate to admit that Im asking this, because I never bothered to ask as I never really cared what the industry protocol is, until recently. I suppose because Im a desperate man now.

Now, Ive changed my mind on the subject. I think auto titrating devices just complicate things and have begun to see things more the way many sleep specialists see them...a single set pressure is oftentimes the best.

I understand what you are saying. But there are sleep docs and sleep techs out there who do know what they are doing. And there are protocols for setting these things up a right way and a wrong way. I know because my first CPAP experience was done correctly, I know it can be done if these professionals do their job.
Im trying to find out the basic protocol for setting up an AutoPap so I dont have to rely upon these professionals nearly as much in the future. Because I am not getting anywhere offline in the last few years and Im tired of it.
I have never been a sheep with my insurance and DME, btw. More like a pitbull. Might be one of the reasons I get treated the way I do...payback.

you do tests, and you can evaluate the data, and use judgment to pick a starting place, and then collect more data, reevaluate, and adjust... and since people aren't precision mechanisms, it's best to keep collecting data and adjusting.
it's easy for anybody that pays attention to see, a sleeper will have good nights, and less good nights, without changing anything on the machine, because there are so many other variables. what one had for supper, what's blooming in the air, what the temp is, what the humidity is, whether one's aches and pains are better or worse, amount of water retention (possibly back to what was eaten that day) and probably hundreds of other variables.
simple and fast protocol for coming up with a number? nope.

According to the PA at my sleep clinic, without a lab titration, they set it from 6-12 and look at the results. There are several reasons for this. The majority of folks will generally fall within this range. Someone who may need higher pressure has time to adapt before determining a higher pressure is necessary. Issues with aerophagia or increased acid reflux GENERALLY is less at these pressure. It surprised me that they actually DO this method, but they have patients who are completely unable to be accommodated in their lab environments due to psychological or physical restrictions.
