What do I have the doctor to write on the prescription for me to have a re-evaluation of my cpap machine?

You don't need a new prescription unless you are requesting a new CPAP machine. What do you want re evaluated?

We would need to know what equipment you are using to make suggestions for reassessing your pressure needs. Suggestions will hinge on if your machine reports data and if it is an auto-titrating machine (apap) or continuous pressure (cpap) machine. If your machine is data capable, you can get your answers here. If it is not, you can ask the doc for a trial on an auto to retitrate. Let us know.

Right!
If you have a data-capable machine (and you should have), you would be able to "reevaluate" your therapy every day.
