It's hard to say what to use for a specific individual, so I'd try a small difference to what you've done recently to see if it helps for e.g. a week or two, then increase it if necessary for longer term.

Find the MD who's the least idiotic and get him to refer you to real specialists to deal with the LPR, gerd and whatever else is going on, maybe an ENT to deal with the throat thing etc... I know it's hard to keep running around, but you have to do something.

When's the last time you talked to your MD in any detail and what did they say (if anything useful)?
Have you considered going somewhere else?
Have you had bloodwork etc. done to rule out other things that could be causing your symptoms?
