nearly new ASV settings
Picked up my nearly new ASV machine today. Just setting it up. On CPAP I have been on a pressure of 8cm, it takes care of my OA's.
I am going with : EPAP 6, Min PS 5, Max PS 10. Will try it for a hour and see what gives...

If your CPAP pressure has been at 8 cm and it has been taking care of your OA's, you are likely going to want to set your EPAP at 8, as well. The 36007 does not have the auto-EPAP, so it will not increase in pressure if you start having OA's. PSmin of 5 might be too uncomfortable and unnecessary. I would start with PSmin = 2 and let the machine figure out what PS is best. Also, set PSmax as high as it will go so you don't limit the ASV function of the Adapt.

I second the notion of leaving the max ps high, it defaults to 15. and it's the high possible PS that gives the adapt the ability to respond effectively to your centrals.StephenR wrote:Picked up my nearly new ASV machine today. the min ps might be alot more than you're used to, but play with it and see how it feels, you might like it. I'd also agree with perhaps a higher epap, since the epap is what prevents the OAs.

Have to say the hour I spent breathing with the machine was wonderful, I have a feeling this is the machine I "should" have been on in a perfect world..

what I'd do is leave it at 8, or no less than 7, and see what the numbers look like tomorrow.
if your apneas are low, then you're probably good.

When I change to my Adapt I was able to use approx 2 cm lower EPAP than I was using on my other machines...so you might get by with lower EPAP and I would for sure keep that option open since we know that your centrals are directly related to pressure and thus we want to use as little pressure as possible to reduce the chance of triggering centrals ....if we can.
I do agree to open PS max to 15 though. The machine will likely need it to deal with the centrals and it won't go anywhere it doesn't think it needs to go anyway. If it doesn't need it then it won't go up there but it might be needed so don't tie its little hands with limited PS.

Automatic is correct. Personally I have found the best results by only really setting EPAPmin to prevent OA's, use a minimal PSmin (2 in my case) and let the machine figure out the rest. It's amazing how well and how quickly the algorithms on these high end machines work. Its when you try to limit them that they don't work well (high AHI) and can actually make therapy uncomfortable (aerophagia).palerider wrote:[

There's no need to use a higher EPAP at this point especially since you aren't comfortable with more and there certainly isn't any urgent need to use more.
I know it's probably too soon to notice much difference in overall sleep quality or how you feel...but you look real good on paper at least.

I just could not go back to CPAP at this point...

That failed ResMed thread thing....those people were already real sick before the experiment and with any treatment there are always risks...even taking that once a day baby aspirin to ward off heart attacks.
With any treatment we simply have to weight the risks vs the rewards.
I look at xpap therapy as a preventative thing...preventing that bad congestive heart disease in the first place is a much better reward than trying to fix it after it develops. If there is the slightest chance that optimal therapy is preventing something bad from developing down the road...then I am all for it. It's preventing further damage/stress to the body and thus there's a good chance it is preventing that really bad stuff from happening down the road.

as to the resmed failed thing, that's apparently a particular subset of a particular condition, a type of congestive heart failure and csr... more details to follow, I'm sure.
