One week in: ? about titration study vs. auto PAP

Post by jon k » Wed Mar 04, 2015 9:22 am

I've been sleeping with my CPAP now for one week, and I have to say that I think I got my best night's sleep last night - I actually had some some sleep, positive vivid dreams (that included moving back to Boulder and barbecue! LOL), and woke up cheerful. On the other hand, I woke up with deep pain from my mask, so I'm headed back to the doctor tomorrow to have another fitting, although, I have to say that I was able to adjust it to where it was tight enough for pillow pressure not to cause a leak but not so tight as to hurt my face, so that's a success too. Here's my weekly summary:

Day 1: get loaner CPAP machine from Sleep Center, sense of relief at night that I can breath
Day 2: wonder about all the settings and notice sinuses getting bad, but still grateful to be breathing at night
Day 3: awful allergy reactions all day, dreading using machine at night
Day 4: Very helpful advice from Pugsy about the humidifier settings; deep relief to my nasal passages that night
Day 5: Better sinuses, mask begins to hurt at night
Day 6: Pain from mask all night, tossing and turning, but still enough energy to be able to work out for the first time in weeks
Day 7: Best sleep yet; was able to adjust mask, and am clear what I need to ask for at my dr's appointment tomorrow.

Progress!

Question: Anthem BCBS has denied authorization for an overnight CPAP titration sleep study; doctor is suggesting auto PAP study at home. Any opinions about that? Luckily, my spouse works at a health care policy law firm, and I about five lawyers who are willing to take this on for me if necessary, but the sleep study center told me that many of their patients do the auto PAP at home. Thoughts?

Thank you all again. Community is an awesome thing.

Go ahead with the APAP titration at home. Did you know that is the way Kaiser does it all the time and they save the in lab sleep studies for people who end up with some sort of problem or complicating factor?
For plain jane OSA most people can do quite well with going the APAP at home route. There's always going to be outliers who don't fit into the mainstream but the odds are that you won't be one of those.
Besides...those in lab sleep titration studies don't always get it right for various reasons (I was on of those people) and I would/could have done just as well with letting the APAP help me find my optimal pressure because that's what I ended up doing anyway.

Try easier and simpler first and then if you end up with some sort of problem then you can worry about having a fight to get the in lab titration study.
