Sleep Study Results In- Need Advice

Hi all,

I'm new to the forum. I had posted on here a few weeks ago with my preliminary sleep results. At the time all I had been told was I had an AHI > 15.

I now have the full results for the study, and a follow-up appointment set on March 13th to be fitted with a machine. The abbreviated results of my study are below:

Total Diagnostic Sleep Time: 104 minutes- I was only able to stay asleep during the study for about 2 hours
Apnea Index: ( 0 obstructive, 4 Central, 0 Mixed) for an AI of 2.3 events per hour
Hyponea Index (HI): Total of 152 hyponeas for an average of 88 events per hour
Apnea Hyponea Index (AHI): 90 events per hour

"No REM sleep was captured during the diagnostic portion of the study, therefore, this may be an underestimation of the severity of the patient's sleep disordered breathing."

"No events characteristic of PLMS. No abnormal EEG activity. No parasomnias. No cardiac arrhythmias."

"Conclusions: Severe OSA, Treatment-emergent central sleep apnea was present"

"Recommendations: BiLevel ST 13/8 cmH2O with a back-up respiratory rate of 12bpm. Recommend ResMed Quattro Full Facial Mask."

Before I go into my appointment on the 13th I wanted to gather some info, perhaps hear from some of the more experienced users on what they think regarding the recommended machine, titration, and mask. I was only asleep for 2 hours during the study. The other 6 hours I was put on CPAP (BiLevel ST titrated at 12/8 cm H2O with back-up respiratory rate of 12 bpm) but could not fall asleep with the mask on. I'm worried that the recommended titration level is not really what I need- as I wasn't even asleep with the mask on- I'm not sure how they can recommend a titration level if it wasn't even tested while I was asleep?

I really want to get this right the first time and not have to play around for months/years getting the proper setup. I'm coming to understand there is so much to learn about all the different machines, accessories, and software. Apparently I won't have a say in what type of machine I'll be prescribed but I can ask for different masks at the follow-up appt. Still, does anyone recommend a particular machine or titration level based on what my results were above? Also, what do you guys recommend I discuss when I go to this appointment? Any advice would be greatly appreciated.

See if your doctor will prescribe an ASV machine instead of an ST machine. It will be more flexible if you develope, as was suggested, pressure induced central apneas. I think the reimbursement code is the same for both so your insurance won't choke on the change.

Okay, thanks JDS74. I'm getting the machine through the VA so I'm not sure how it works with them. I'll ask about the ASV, hopefully they're willing to work with me.

Sometimes you have to do a lot of failing of other machines to get ASV. Fortunately for me, I failed BiPAP at my titration (caused centrals, raising my AHI to 9 something), so my insurer allowed ASV. I'm scratching my head as to why a regular BiPAP when they know there is central apnea, but stranger things have been recounted here.

You only have a say in your equipment if you say your say. Although I'm sure the VA is a difficult setting.
