New User - High AHI

I just received a new APAP machine about 3 weeks ago (was on CPAP for 4 months before that) and thought all was well. I finally downloaded SleepyHead to see my AHI > 12. Can someone please help me on where to start to lower this number? I can send whatever info you need.

Need to know the AHI break down into each event category...not just the total AHI.
If high centrals...need to have a talk about centrals.
If high OAs then maybe more pressure depending on what you are using..cpap vs apap with a range of pressures.
So we need more information to be able to help you better.

Which machine did you get and what are your pressures?

Is it model 550 or 560? Model number is on the bottom of the blower unit. Right now you have chosen a 550 model but the 560 model has been out a couple of years now.

It a 560P. Range is 4-20. On my old machine it was 13. I also wake up a couple times a night so I'm assuming that doesn't help. Tell me exactly what you need and I'll get it to you. Still getting familiar with the terms so I really appreciate your help.

I suspected that they gave the apap to you with wide open range. That's easily fixed.
While you may not need 13 all time that 4 cm likely too low and just need to increase it a bit so the machine has a better head start on preventing the airway collapse. It just can't respond that fast.

If you have a 60 series machine then this is the correct choice in the equipment menu
PR System One 60 Series Auto CPAP....note that it mentions 60 series where your previous choice does not.

Need a screen shot of a nightly detailed graph from SleepyHead...pick a typical night.
If you don't know how to do the posting of the screen shots we talked about it here and there's some examples

If your AHI is primarily OAs and hyponeas...then you just need a little more minimum pressure. How much more I don't know yet without seeing what the pressure line is doing.

If your AHI is primarily ClearAirway (Centrals) we have a different discussion but won't cross that bridge unless we need to. It's normal to have a few...they are only a problem when there's a lot more than a few going on all the time.

Julie if you click on one of the tiny pics it enlarges to full size.

This image tells me all I need to know for right now. I don't know what to make of the Clear Airway events at this time.. They might be arousal centrals from all the OAs. Worry about them later if they don't reduce if the OAs are under better control.

That 4 cm minimum pressure is no where near what you need to prevent the airway collapse. I suspect that you need at least 10 cm minimum pressure and that goes along with your past history of needing 13 on cpap. You may need more than 10 cm but that's where I would start if it were me. Try 10 cm minimum and see what the reports look like. Might need more but with things the way they are right now it's all over the place trying to play catch up and can't give us a better idea until the pressure line gets more stable.

Thank You very much! The old machine had a ramp up which also started at 4. Probably why I haven't felt any better since starting about 5 months ago. I'll make the necessary change. When should I post updated data?

I have that happen to me sometimes too. That's why I went ahead and brought it over here for easy viewing...just in case clicking on it didn't work for you or someone else.

I have been having trouble viewing images off and on for a month or so. People see stuff I can't see or enlarge and I can get stuff enlarged that others can't. Computer gremlins is all I know to blame it on.

You have to go into the ultra secret clinical setup menu

Is your machine a 550 model or 560 model (it's on the bottom of the blower unit with maybe DS or REF in front or behind)?
The reason I asked is that the machine you have currently chosen is a 550 but those were replace by the 560 (60 series machines/humidifiers)...so if it is a 560 your profile equipment choices for machine and humidifier are incorrect.
You would need the choices that have "60 series" in the model name.

Once you determine exactly which model you have you can request the clinical manual for it from here. Lots of good information in it that isn't in the user manual.

Edit: I see you already said you have a 560 machine... when you have time you might want to change your equipment profile to avoid confusion later.
