Titration Study? 3 month follow up soon

Post by Amenite » Sun Aug 31, 2014 8:44 pm

I have three month follow up soon and I am guessing it will be with my doctor's Nurse Practitioner (or whatever her credential is). This is the same office that told me they use only the compliance data and don't bother reading any of the other collected data (PRS1 460). So I am assuming they'll try to get me approved for a lab titration study and I guess I would do that if insurance approved, but I am thinking about a new Doc since that bit about the data card being for compliance only. I will walk into this appointment as I did my last one knowing way more about my treatment than they care to look at.

My AHI is < 1.0 for almost three months on 14 cm pressure and I am totally comfortable lowering it on my own very very gradually to find my "best" pressure.

If your AHI is less than 1 with a pressure of 14...that's very close to your best pressure.

why do you want to take something that is working, and muck around with it till it's not?

I would like to be at the lowest pressure that works. Considering where I'm coming from with AHI > 100 I don't see any risk in lowering it to 13cm for a week, or just for one night if the AHI goes up then put it back up to 14.

I was titrated at 14, and on an auto machine, I rarely go over 12.6.
I think the labs sometimes add a couple points, just to cover their hineys.
These stooges have no idea what misery they inflict.
I pray for karma . . .

what would a lower pressure get you? are you having problems where you are? aerophagia? leak issues?

are you trying to fix something, or just want to play?

No I'm not trying to fix anything. If one or two cm lower works about the same I would want to know that. I'll see what happens with this next follow up and if they get me into another study, and go from there.

I think just about everyone's goal is effective treatment at the lowest pressure. Just a point or two can make a difference in comfort, leaks, belly air, mouth leaking, etc. For those who are pressure sensitive, it can make a difference with causing centrals. I say why borrow trouble by using a higher than necessary pressure if it's not needed to achieve effective treatment? Your AHI is enviable now - looks like you've done a good job of honing in on a working pressure. If you do try a lower pressure, you may find yourself in a position of having to decide if the advantages you see using the lower pressure are worth the trade-off of a slightly higher AHI. Or maybe you'll get lucky and get the same results from the lower number. Hopefully soon you'll be settled in and able to just enjoy your sleep, satisfied you're getting the best treatment this technology can provide. Let us know how things go with the doctor.
