First Week - went from AHI of 1.7 to 10.5! Help me out?

Post by clay551 » Fri Aug 29, 2014 8:13 am

So, I'm only a week in with my CPAP therapy and I'm getting concerned and was hoping someone might have suggestions or info for me. The first 4 nights were awesome! No issues, AHI would go between 3.5 and 1.7 but the last three nights I have been hearing leaks (nasal pads) and having to adjust often. Last night I was waking up all the time and this morning saw that my AHI was 10.5 - I never took the mask off and, to my knowledge, didn't open my mouth. Is it common to have one night amongst many that maybe your mouth opened resulting in a high AHI or should I talk to someone about my mask, etc? I have a headache today and can definitely tell I didn't sleep well. I'm just sure if I'm over-reacting with being this concerned or what. Any suggestions/info? Thanks

It would help if we knew what equipment you were using. How about adding your equipment to your profile like I have mine?
Don't know how?

Depending on which machine you are using if the bulk of that AHI happens to be centrals then it's very possible that the machine flagged awake/semi awake breathing irregularities as central. It can't tell if we are awake or not.

So before panic sets in about the AHI ...the event category breakdown needs to be evaluated along with when those events happened and looking at the daily detailed report using the software is the best way to try to figure it out.

How to post images of your reports so we can see what you see is explained here if you don't know how.

Pugsy - thanks for the info. I will definitely update with my equipment once I get home from work and look at what model, etc. the machine is. Thanks again for the links - it's greatly appreciated!

Pugsy's response is the technical one, which is worth plenty, but I will give you a practical one based on my experience. I started CPAP on Feb. 3. My first 3 nights were greater than 5 AHI, with one of those nights at 21.72 AHI. In my first three weeks, I had 6 nights greater than 5, and in 6.5 months I've had a total of 8 nights greater than 5. Since April 10, I have not had a single night over 5, with most of those nights less than 3. So part of your problem may be the learning curve that we all go through when we begin this journey... but the details Pugsy asked for may provide some insight, so be sure to post the information she requested.

My first week, I knew nothing about how to adjust for leaks, which were my main problem. I was glad I didn't have a bed partner, as I sounded like a whoopee cushion most of the time. When I came here looking for answers, I learned about mask liners. Are you using the Gecko nasal pads? Your mask could be leaking where the pad stops as there would be a gap right below it.You might want to switch to mask liners instead. In addition to helping with leaks, liners protect your face from sores and irritation. Mask liners were a game changer for me, and I will not go without them! Karen at Pad-A-Cheek makes nice ones, go here: http://www.padacheek.com, or you can get RemZzz liners from our forum sponsors at https://www.cpap.com. RemZzzs are also available at many DMEs, and I believe you can get them at places like Amazon.

Now using AirFit F10 mask; Quattro Air is backup mask. RemZzzz mask liners with both.
