I have private insurance, not Medicare, Medicaid. I have in the past been reembursed for purchases from cpap.com at out-of-network rates. As the page you provided states:
The pre-populated form may be used to seek reimbursement from your insurance company. CPAP.com is out of network for private insurance companies.
Which matches my examples. I stated up front, I was providing an example that I had recently used.

My comment: That is not how it works was related to your comment:
If YOU submit an invoice to your insurance provider, then YOU are a "vendor". If you're also an insured member, then it should be logical that YOU are in-network as well.
But you are NOT the vendor. You are submitting an invoice to your insurance provider "from a vendor which is out-of-network". Which is why I responded that is not how that works.

Anyone submitting a valid invoice to a company becomes a "vendor" in the accounts payable system for the purpose of payment.
