Help with New 960

Help with New 960

Good morning..

I need help understanding how the four pressure settings interact.
My current settings are:
Max Pressure:25
EVAP min: 4
EPAP max: 20
PSmin: 5
PSmax: 19
BPM: Auto
Bi-Flex: Auto
With these settings I get exhale: 5 cmH2O
Inhale of only 9 cmH2O

I cannot figure out why I cannot get the inhale pressure higher, if I lower the exhale to 4 cm then the inhale drops to 8 cm.
Also I seem to be unable to set the PSmin higher that 5 cm.
Hitting this setting wall worried me at first, my old 950 running at a inhale pressure of 9 cm would make me feel short of breath, I had to set the 950 at 12 cm to not feel starved for air.
But I looked up my Doctor’s titration report and 9 cm was the best results so I tried it and found with the 960 I had no shortness of breath.
So I was OK at these settings.
But I fear I may need more and want to understand how to set these things.
OK Last night seems to have gone fairly good, but is these readings alright?

Note how the mask pressure when up during the sleep cycles, any suggestions?
I also cannot seem to find Sleepyhead’s O2 recordings, it will upload and will show the graphs but once closed they disappear and if I try to open them again they seem gone. I am having to use another program to keep these records.

Your first graph says your "inhale" 95% percentile is over 14 cm h2o. I'm confused why you think it's only having an inhale of 9 cm h2o? Are you talking about when you're looking at the screen when you're awake?

Your pressure support minimum is 5. This means that your "inhale" pressure has to be AT LEAST 5cm h2o higher than the "exhale" pressure. When you're dropping the "exhale" pressure to 4, the "inhale" pressure still needs to be AT LEAST 5 cm h2o above the "exhale" pressure.

You said you lowered the exhale to 4 but then inhale was only 8 - im confused, you've said your settings were 4 on exhale already?

I think you are a bit confused about the terminology. Both those "EPAP" values are exhale, and both the "PS" values are inhale.

OK that improved my efforts on settings BUT things are still funny..IF I do what I think is a good setting, things go sideways...

I felt that on EPAP min at 4 cmH2O and EPAP max @ 20 cmH2O To allow it to go as high as it may want

Then I set PS min @6cm and PS max @ 20cm I do understand this min setting is what it will run above the EPAP Min so 4 + 6 +10cm. And again I set 20 thinking that would allow the system to go as high as it may want.

BUT when I did that I lost PS min which now read 0 zero.

Well it wanted to run a 4cm for both EPAP and PS..

I found I have to lower the EPAP max and PS max to EPAP 18cm and PS max to 15cm be allowed to reset things so it will work.

I am hope someone can explain these settings.

At last I was able to get things running where I wanted them at 4cmH2O on my exhales and 10cmH2O on inhale.

I left the system BPM on auto.

This does seem to be working well,

Some odd pressure readings BUT I feel good so I am thinking it is doing what it is suppose to do..?

I don't know exactly what settings will cause other settings to reset due to limits, but what would help is if you could say:

What do you want your actual EPAP and IPAP to be normally?

Just that, no other conjecture or explanation -- those are clouding the issue and making it hard for me to figure out what you're trying to accomplish.

Once I understand that, I can walk you through the meaning of the various settings.

I have it now set to do what I think is my best settings.

A exhale pressure of 4 and a inhale of 10.

I just changed it last night to 5 and 11.

Had a slightly better night.

Trying to fine tune my new Sleep apnea machine...It really seems to be critical to how well I feel during the day.

I think that I must keep my O2 levels above 90%, and the higher the better.

I now remember how my mother was when we took care of her during your last couple of years, she had a O2 compensator, and when she did not remove it during the night she was fairly bright and aware, but when she did remove it she did not know where she was or who I am.

As she removed it most of the time it got pretty bad.

I wonder if this might have something to do with Alhimizers?

So EPAP=4, IPAP=10.

That's EPAPmin=4 and PSmin=6, since IPAP = EPAP + PS.

Assuming that you're on ASV because of centrals or complex, they generally leave the max numbers wide open:

Max pressure=25
EPAPmax=25-PSmin=19
PSmax=25-EPAPmin=21

But you might find those swings too aggressive and unnecessary, and so you could lower those maxes if you don't need them that high. What kinds of apneas are you trying to treat?

I do recommend setting up your ramp in case you need temporary reduction in your settings.

I don't think you can use ramp at those low pressures. Ramp up from 4 to 5?
