Pre-Diabetes and OSA
I have recently (4 weeks) been diagnosed with both Pre-Diabetes and OSA. My numbers during my sleep study were very high AHI 72 while sleeping on my side and AHI 97 while sleeping on my back. How much (if any) can severe OSA play on your body developing diabetes? (I have since started a cpap trial, my first night dropped to 2.3 AHI and I have consistently been 0.40 to 0.80 AHI) Thanks.

I have not been diagnosed with any diabetic issues but definitely have problems as when I eat sweets, I get severe spikes on the glucometer. I am dealing with pap therapy intolerance issues which is a whole other post.
Anyway, if you haven't done so, you might want to purchase a glucometer so you can see exactly what foods cause problems and which ones don't. I hate to admit it but I had gotten away from doing this but am forcing myself to get back to it when my strips arrive tomorrow.

OSA is definitely a contributing factor to pre-diabetes, because poor sleep interferes with hunger regulating hormones like ghrelin and leptin, and blood sugar regulating hormones like insulin and cortisol. Unfortunately, treating OSA is not likely, by itself to "cure" pre-diabetes because along the way you may have picked up other contributing factors including weight gain, use of statins (that lovely class of drugs that's useless in primary prevention of heart disease but definitely associated with diabetes and heart failure), poor diet, and more.
49er's advice is good, controlling your blood sugar is key. For dietary recommendations see http://www.dietdoctor.com/LCHF. Be wary of so-called "diabetic diets" which push you to eat lots of carbohydrate rich foods and processed oils. If you monitor your blood sugar you will see why that type of diet is such a disaster. Carbs are sugar, even if they come in the guise of a so-called whole grain.

I was prescribed metformin and have been taking that as well as monitoring my blood glucose quite religiously (before meal, two hours after and before and after bed, everything seems normal although it's probably the metformin doing most of that. I have been managing my intake dropping soda, sweets, cutting carbs down a lot eating more healthier.

OSA was the culprit in my metabolic syndrome (the less friendly name for "pre-diabetes"). Once my OSA was under control my insulin resistance disappeared, all subsequent testing was uneventful. My PMD was incredulous, he'd never seen anyone actually get a diagnosis of metabolic syndrome reversed, let along by a simple APAP machine.

It may be important to note that some of the more recent info on the question of CPAP reversing metabolic syndrome (info that has made its way, uh, "into the literature," shall we say?) has been retracted upon further investigation.
