Increase in AHI since I quit caffeine and started T3

Post by obrienj » Sat Aug 16, 2014 12:08 pm

Hi,

on cpap for 2 years. Pressure of 14(fixed) for last 18 months. AHI very consistent at 2 to3.5 after initial tuning.

Was diagnosed with Reverse t3 dominance 2 months ago and have had the folowing changes:
1. No caffeine of any variety and I used to drink 4 to 8 cups a day
2. 40mg per day of t3
3. Strictly to bed at 10pm every night. Used to be 1030 week days and 1130 weekends.

in the last 4 weeks my AHI has risen to 6.5 and I'm feeling unrested again. My consultant can't see me until September. 0% large leak on cpap and getting 8 hours per night on it. Had cpap serviced lately also.

Appreciate it if you had any thoughts.

Thanks

What is your machine? There are manuals on line that will allow you to adjust the machine pressure.

If you are lucky you might have a machine that is Sleepyhead compatible. Then you can see the details of your night.

What's the model number?
550 or 560?
If either one of those you can use SleepyHead software or Encore to see what is going on.
Tell us the exact model number (it's on the bottom of the blower unit and is a 3 digit number which may have REF or DS in front or behind it) and once we determine exactly which machine you have you can add your equipment to your profile so that we can easily see what you are using and won't have to ask all the time.

Remstar Auto AFlex covers 4 different generation of machines. Need to narrow it down a bit. Since you mention Phillips Respironics I am assuming a PR System One model and the only ones with AFlex are the 550 and the most latest model the 560.

The model 551 is the same as the model 550 in the US...the xx1 thing denotes probably either Canadian supply or maybe the UK.

This would be your correct choice for the machine.
PR System One Auto CPAPxxx
and the humidifier would be
PR System One Heated Humidifier.

It is compatible with SleepyHead and I think I already sent you a PM about Encore. If not let me know and I will get it to you.
Encore Pro can be a real PITA to install and use...so if you have trouble we may want to use EncoreBasic. The reports are identical to Pro's reports but it is MUCH easier to install and manage. So just let me know.

Let's use the software to compare the old AHI and the new AHI to see first of all what kind of events you are having. It may be that they are Clear Airway events (centrals) and we just sort of shrug our shoulders and watch them.
It may be that you need a little pressure adjustment if they are obstructive in nature.
As to why the change? Beats me. Sometimes crap happens. Sometimes people change sleeping positions (supine can make OSA pressure needs increase) or maybe getting deeper sleep and thus more REM sleep which can also cause an increase in pressure needs.

But first thing to do is use the software to see exactly what that AHI is made up of before we start worrying about it.

SleepyHead's Windows version works on both 32 and 64 bit computers up to and including Windows 8.1
http://sleepfiles.com/SH/
There is no installer yet for this trial version so after you unzip the folder you have to go into the main folder and scroll all the way to the bottom of the files and find the SleepyHead application.exe file and click on it to open and run SleepyHead.
If you have trouble you can click on the link in my signature line that goes to Robysue's instructions for using. I think it is the bottom link.

What I did was just right click on the application.exe file and pinned it to my taskbar so that I didn't have to go into the main folder each time I wanted to ope SH. You can create a shortcut to your desktop if you wish too.

If you have a Mac...I don't know which Mac OS is supported but we have lots of Mac users using SH and they can help you out.

See this thread for information on how to post images of your reports.

The nightly detailed reports are better for viewing than the long term trends are.

Hi Pugsy,

I decided to increase my pressure from 14 to 16 on the basis that whenever my AHI crept upwards previously a prescribed increase in pressure did the trick. My consultant can't see me until September so hence the self-medication.
also, my obstructive apnea events had risen considerably so I put 2 and 2 together and thankfully it came to 4.

The results have been very strong: my AHI was averaging at 6.7 and the last two nights have been 1.07 and 2.1 which is excellent.

I'm using your sleepyhead software which is a fantastic piece of work.

Thanks for your help.
