Charle, you're going about this very methodically. That's good.

With your Vantage autopap, are you using the "Mirage" mask setting? Just guessing, I'd say that would be the mask setting to use for the Aura mask since that's the correct setting if a person were using a Swift mask. Both are nasal pillows interfaces.
I may switch over to CPAP mode after a few weeks to see if I can bring the AI & HI numbers down.
What was your AHI (or RDI) from your sleep study, and what was the prescribed single pressure from the sleep study titration?

Charle

Rested Gale makes a good point. The right masks setting can make a difference in treatment as well on some of the numbers that are reported. I called them masks curves and I did noticed that the curves made a difference in when does the Spirit reacts to flow limitation for example. Don't forget that there could be two Mirage mask setting and one is the Ultra Mirage. I not sure that the new S8 is keeping the older Mirage mask setting but I find that the Mirage setting a good starting point better than using the standard setting for a unknown mask. The Mirage setting is in the middle of the ball park of all the masks setting on my S7 Spirit as far as vent leak levels goes. Sometime if someone could upload the new mask flow/pressure characteristics chart for the S8 I am just curious.

John

Pantyhose Trick

Post by snowloft » Mon Oct 10, 2005 12:21 pm

What is the pantyhose trick for the nasal pillows? I have small leak issues I think and would be interested to find out. Thanks

john5757,
Thanks for your post...
I can't upload the pressure vs. Flow Curve but I can give you the numbers that make the curve.
AEIOMed Vent Flow Rate Curve:
Pressure (CM H2O) vs. Flow Rate (LPM)
4 - 14.5
6 - 18
8 - 21
10 - 23.5
12 - 26.5
14 - 28
16 - 30.5
18 - 32
20 - 33.5
I've looked online for a download of the curve and failing to find one, I hope the numbers above are of some help.

I'm extrapolating the values by looking at the chart but they are probably within .5 LPM of the value. I hope that this helps. BTW, I've set my S8 to the mask setting that corresponds to the "Mirage Swift Nasal Pillows System".

snowloft,
Take a look at this previous post on the subject of tweaking the Aura. I've finally been able to get a good seal by using the "green band addition". I've been able to sleep on my back or side without any perceived leakage at the nares. Good Luck!

Charle,

What I meant to say that in the manual of the S8 there should be a graph of all the different mask settings that plot the pressure with the flow rate. You have given me your mask leak rate and that telling me that the Aura Nasal mask has a low exhaust port flow rate which is even lower than my Deamfit mask that I am currently using and lower than all of the ResMed masks that I am aware of. My S7 Spirit does not have a swift mask setting. That is just what I was looking for but it is not a big deal.

UPDATE:
I compared the vent flow rate of the Swift with the Mirage setting on the S7 Spirit they are very close to each other.

Thanks for posting graphs under the FAQ but howerver the vent leak rate between the swift and the Aura are very different from each other. I check this three times already.

Geez, John, I don't know what you're talking about, but that's okay. As long as Charle does, that's what counts.

Sleeping better than in years...

Post by Guest » Tue Oct 11, 2005 11:21 pm

I've finally got the leak rate down to a very low rate of 0.04 L/S. Taping the mouth and the headband addition to the Aura (to help keep it firmly against the nares) finally did the trick.

My pressure was 11.8 but my AHI is still up around 14 (4 AIs). Despite the AHI rate, I haven't slept as well in years. My wife loves the tape over the mouth and reports that the snoring is gone as is the thrashing. I still have an occasional twitch but that is it. I've also found that I'm sleeping through the night now but best of all... my wife has slept better these last three nights and is feeling much more rested as well.

I'm very happy with the results so far and am planning on eventually switching to CPAP mode with a pressure of 12 and an EPR of 3. I don't know if it is too much to expect but I'd like to bring the apnea and hypopnea incident rate down.

Charle

That is good that you got the leak rate down and everything is setting down and your wife is sleeping better as a result of you being on CPAP. Unless you download the data from the machine to a computer the reading on the display of the machine is the maximum pressure not the 95% pressure. This means that the idea pressure for the CPAP mode may be a bit lower like 11 not 12. Just my opinion only. I am sorry for the confusion for the mask setting the bottom line that I was trying to say was that the Swift and the Aura mask graphs as shown on the CPAP FAQ here do not match with each other. By the same token you are doing very well and I hope for your continuing success.
