You should be able to make your own. Write up your name, address, phone, next of kin's contact, your PCP's contact, allergies, family medical history (heart disease, diabetes, chronic illnesses, OSA, etc.) medical history (for example, I had mumps, chickenpox, red measles and german measles as a child), surgical history, GYN history for women, medications (xPAP settings and mask), supplements taken and anything else you can think of. Get copies of your last comprehensive labs and EKG from your doc. Tell the doc what you're doing and ask what s/he would recommend be included. Take the whole mess to Staples, Office Max, Kinko's, etc, and get them scanned and put on a mini-CD. Put the CD in your wallet (but within a protective case of some sort). On a photocopy machine, shrink the history papers (mine's 3 pages, size 12 font) down as far as is possible and still legible and put that in your wallet/purse, too.
