sleeping 6pm to 3pm the next day WITH CPAP!

Post by MTmama » Tue Mar 11, 2008 4:15 pm

I am so tired and my CPAP is not cutting it anymore. I have an appointment to go see the doctor soon about getting an APAP (pray pray pray I don't need another sleep study) but in the meantime need some relief from this god-awful sleepiness. I slept from 6pm to 3pm the next day!

I'm also really depressed and on a few medications and my first instinct was that it was the medications but it's been 2 years since my last sleep study and I've gained 100 (yes, 100) pounds since then. I was set on 9 and have tried having it set all the way up to 14 and still felt tired.

What can I do to get some relief in the meantime? Sleep in a different position? Add pillows? Raise the head of my bed? Change my machine setting myself? (tried that already and it didn't really work...)

Any suggestions?

Also, is there a certain type of mask that would be better for really severe apnea?

Please fill out your profile so we know what you ARE using.
If you have a nasal mask, you may be leaking your therapy air out your mouth at night. That would be one reason to account for how you're feeling. If you're leaking air, it doesn't matter what your pressure is set to......most of it will be going out your mouth.

On another thread of yours you wrote
MTmama wrote:Yes I am aware of that, and I've brought up that fact to my psychiatrist many times. However, right now, considering I'm borderline suicidal, they insist it is the best treatment for my anxiety at night. I have severe major depression, bipolar II, borderline personality disorder, and obsessive compulsive disorder. They are weighing the risks versus benefits.

The kind of meds you are on increase fatigue, probably cause weight gain, and who knows what other havoc they are causing.

I suggest you call your doctor and demand to be seen. If that means you going inpatient for a few days, then do it. I suspect there is a lot more than just OSA behind what is going on. Getting in the hospital might just be the way to get the sleep apnea looked at faster.

It sounds to me like CPAP isn't doing (or cant) do it's job. Are you mouthbreathing? Do you have a data capable machine that can tell you your morning AHI?

You could wear it 24 hours a day but if the pressure is wrong, the mask leaks or you mouth breath (without a full face mask) it means absolutely nothing.

Without data to see what's going on, changing pressure is just shooting blind...in the dark.

An APAP would take care of that for you but in the event you can't get one you should at least try to upgrade to a data capable machine so you can track your own progress. Failing that, you would have to have another sleep study to see whats up...especially since you have gained so much weight. That could be the entire problem.

I am curious why you "pray pray pray you don't need another sleep study". It's just a night with CPAP...like you are doing now, and should be used to.

I go to the psychiatrist tomorrow and am talking with her about starting Provigil to offset the sleepiness as well but the majority of this sleepiness started before the meds, too. I think it's really a combo of the depression, meds, and OSA but I know that my sleep apnea is really a huge huge part of this. I think I may be leaking air at night.. I use Mirage Swift II and the air coming out is pretty loud.

For the record, now I am just taking Paxil and occasionally Xanax if I can't sleep but that's it! (until tomorrow... I'm sure the med rollercoaster will continue with something new...)

a new sleep study is hard for me because I am a single mom of two toddlers and don't have anyone to watch them so I can go. Otherwise, no its not that bad.

It shouldn't be loud. There's a leak. Maybe a fitting check with your resp. tec would be a good idea.
I found I had to increase the size of the pillows after a while with the Swift.

I wear the Swift original pillows, but I too discovered that I get a much better seal (and better numbers) from using the large pillow. I just assumed that because I have a small nose, I would need a medium, at most. But that wasn't the case. And I agree, your meds could be causing all your problems.

The Swift pillows is a noisy mask, but what people keep asking you is whether your mouth opens when sleeping, and if it does you need a different kind of mask altogether, because noisy or not the mask you're using (or any nasal mask) is a waste of time and your energy. If you're losing the 'fed' in air out your mouth, you need a full face mask if taping doesn't work.
