Bi-level Synchronization/Sensitivity Settings

I am hoping the folks here at CPAP.com can help.

I spoke with a clinician at Resmed today and told her of my "Brain Fog" issues with my therapy. No Snoring, No Arousals, just a general feeling of disconnect.

There are sensitivity settings on The EPAP and IPAP on my Puritan Bennet GK 425 as well as a Rise Time (RT) setting that cannot be self adjusted. The purpose is to synchronize breathing with the pressure change. She was taken aback that DME left these at the default settings. They need to be tuned to a persons breathing and with the interface they are using. A full face mask requires a more sensitive setting because of increased dead space in the mask.

As a result, I may not be having Apneas, but, the sensitivity settings may be causing Hypopneas.

Anyone know how I can find out how to tune these settings? It seems to take like 1/3 of a breath for the machine to realize I am inhaling and I may need to increase the IPAP sensitivity a tad. My DME is not returning phone calls, and pretty much...if I don't get rid of this brain fog, my livelyhood is in jeopardy!

Dr wants to put me on Provigil, but, my reseach shows that it doesn't do squat for cognitive function. Besides...using a drug to cover up the effects of sleep events is what I did back in my pre-diagnosis days, where I ate Ephedrine tabs and washed em down with a large cup of coffee!
