Suggestions ? On settings for ResMed Aircurve 10

Post by cvmonk » Fri Jan 19, 2018 12:10 pm

Does chart indicate all central? Only says “apnea”
Mode = ASV Auto
Min Epap 5
Max epap 10
Min PS 3
Max PS 20
Using Rescan 5.9

So two questions
1. Central vs obstructive? I can’t tell. I think all central
2. Suggest any pressure changes?

You need to keep to one thread instead of starting new ones with same problem. If no response to one just give it a bump to bring it to the top of the page. Here's your prior thread that ties into this new thread.

ResMed ASV machines don't distinguish between central apneas and obstructive apneas...you just get "apnea" and of course hyponeas.
Generally if the machine doesn't have its hands tied to where the machine can't breathe for you if a central happens then it is assumed that the apneas flagged are obstructive apneas and not centrals.
Your machine has PS max of 20 so its little hands aren't tied. It can and will respond to centrals and breathe for you.

I assume you meant EPAP min of 5 and max EPAP of 10 and this might account for the clustering of what we assume are obstructive apneas.

Your clustering....are you on your back when you see the clustering...do you start out the night on your back?
If so that would greatly increase the chance of those apneas being obstructive and simply needing more EPAP pressure to prevent the airway from collapsing.

Is there some reason you aren't using SleepyHead?

If it were me I would look at sleeping position affecting things and maybe consider more EPAP minimum and maximum.
Unsure as to how much though...probably at least 2 cm each and maybe more.

I start by sleeping on my back.
I started with the resmed program long ago. Is there an a advantage to sleepyhead ?
I posted a new one because I didn’t have an image to begin with and settings have changed. Thanks for the input

SleepyHead makes it a bit easier to zoom in on selected events but the data shown is the same data as you see in ResScan.
There are some statistics available in SleepyHead that isn't available in ResScan....like how much of the time do you spend at max EPAP of 10 during those ugly clusters. It would help in figuring out does minimum EPAP need to be increased a lot or could we maybe just let max EPAP go higher and see if it helps or not.

I suspect you need more EPAP pressure when you are on your back...your obstructive apnea stuff is worse and more stubborn when on your back.
EPAP is generally what is used to hold the airway open.
Could it be accomplished with just raising max EPAP???? Might and might not. It's painfully obvious that it isn't working well now when you are on your back.

You might read this and get an idea how EPAP for obstructive stuff it's the most important setting. While it's a Respironics document the basic premise for how to deal with obstructive stuff is the same no matter which brand.

Here's an example of part of what can been seen on the ASV SleepyHead report.
Looks at the statistics on the left side. There's more but this one I just happened to have handy and it's just the one night's detailed report showing the basics.

get some sleepyhead charts up, it is an easier program to use. You will find that they are marked UA, but all UA are OA, because the CA is being treated. You will find that raising your min epap will clear these events along with positional therapy. It's a matter of seeing which is which, as you go along. Be aware of chin tucking towards your chest, some need a $10 soft foam cervical collar to straighten the airway.

The resScan program need the display settings changed, so it's easy to see. You could put up the statistics page. I would suggest you set min epap on the median pressure and adjust it as you go. I would also raise max epap to 15, to give the machine room to move.
