Do you have the sleep study? Your doctor will want a copy and I know your insurance will need it. Also the DME will need a copy of the sleep study as well. Now the only thing I don't know is if there is a time limit on them excepting the study. What I'm say is if they never had a machine I don't know if the sleep study would have to have been within the last year or how old it could be.

The OP may...or..may not have insurance. He has not indicated.

Are you going to be trying to use insurance or is the RX just for being able to buy online from places like cpap.com?
It makes a bit of difference on the wording.

When using insurance and brick and mortar DMEs you would likely need to be very specific about the machine you want so I would suggest that you decide on a machine brand and model and name it specifically and maybe include model number.

For masks...the best wording to cover all bases is "mask of patient's choice".

Now if you are just wanting the RX so you can buy from some place like cpap.com that requires a RX on file for cpap stuff that require a RX...the RX doesn't have to be so specific because you get to choose what you want vs a DME trying to choose for you.

You can get an apap with a cpap prescription online...because an apap is a cpap with additional modes of operation offered.

So for cpap.com....this wording is sufficient
"cpap with heated humidity and mask of patient's choice"...that's all that would be needed and you get to choose whatever you want.

The sheer amount of info on this site makes it a bit tricky to search out the best advice. If doc will do it, she'll probably be amenable to wording suggestions. We're wanting a prescription that will give the most flexibility and broadest timeframe possible in selecting machine (APAP of course), masks, and supplies. Any tips what it should include/not include?
My doctor sent a prescription directly to the DME for insurance Purposes. I however wanted my own to keep with me to use when and if i ever needed it.
I recommend you Download this prescription form from Cpap.com and have your doctor sign it.

If you need one for insurance then your going to want to have the Doctor be very specific so that you get at min a Full data Apap from the manufacture of your choosing.

you can read the Cpap.com Prescription Faq here for more information. https://www.cpap.com/cpap-faq/Prescriptions.html

My PCP wrote my prescription, but with some guidance (and a canned cpap prescription form) from the home sleep study company that he used. On the prescription form he filled out they had a selection for "99 months" for the expiration. The form also said on it that that selection meant "forever" in the industry. My DME confirmed that this is an industry standard meaning a lifetime prescription. My insurance (Athem Blue Cross) was a-ok with that (and confirmed that it was lifetime). You'll have to check with yours to see if your insurance would except that. Obviously if you can make that happen, you'll save yourself some future pain.

Note also that if you have a really big (or even not so big) deductible (mine's around 5500 bucks), it may make no sense to even file it with insurance unless they can get you a killer deal on price. Chances are good you'll be able to do just as well on your own. If you've got a very low deductible, or already have met that this year... then maybe it's worth it to deal with insurance.

Download this one, print it and get the doctor to sign it.

If you want it to be more generic, block out the "Supplier", "Supplier Information" and "Fax to" information at the bottom of the page before printing it.

So there is an insurance company that would pay for something a doctor didn't order for you. Seems hard to believe but maybe there is some out there that will just by anything you think you need

Some so-called Cadillac plans cover stuff most of us only dream of . . .
Just saying.

since you seem to have lost the thread of what YOU said... I'll remind you... your assertion was that the OP would have to dredge up their old sleep study because "I know your insurance will need it".

no, they may be satisfied with a doctors prescription, it depends on the insurance company. if you stick around for a while and *read* you'll see tails of peoples doctors writing prescriptions *without a sleep study!* :le gasp:

point being, you cannot speak for all doctors and all insurance companies, so, it's best you don't.
