what's the best machine for an AHI 40?

Read a lot about different cpap machines here. Many people suggested to use the airsense 10 autoset for her/Auset. But if I read correctly, the pressure setting for these are only from 4 to 20 cm. I know the dream station b-pap can set up pressure from 5 to 25. Is that the same setting for aircure 10 bpap too? For an AHI = 40, is that better to use a bpap machine? I talked to my sleep doctor about Apap since I thought it would be more comfortable. But he thinks that's not the one for my AHI. Any brand is better than the other one?

Let's see exactly what type of apnea events you are having before worrying about which machine.

AHI isn't the guiding force for any particular machine but what makes up that AHI could make a huge difference in the type of machine you need.

Wasn't there some questions as to centrals in your sleep study?
If you had a lot of centrals during your sleep study then it is very possible that cpap/apap or regular bipap (bilevel) is not the best way to go.

Do you have a copy of your sleep study done without cpap...we call it the diagnostic sleep study?
If you do...how many centrals?
If you don't have it...get it.

Your AHI is not the determining factor for your actual pressure needs. You could have an AHI of 150 and still only need a pressure of 12cm to correct it. Lots of people do not understand that - your AHI does not determine what pressure you need. I have an 'official' AHI of 5, but need auto-pressure of 12.5. This is higher (and lower) than other people because we are all different.

Your BEST bet is to get a machine that you can read with Sleepyhead. An Airsense for Her gives you a choice of 3 different algorithms - straight pressure, and two different algorithms for autopressure. A Respironics machine will do as well for MOST people. Resmed and Respironics are the top machines, so you can't really go wrong with that, BUT some are bricks (i.e. only give compliance data and nothing useful). The Airsense for Her (which I have) works great with Sleepyhead.

ah ha... my wrong impression again, I thought the higher AHi will need higher pressure setting since I got a 17 IPAP and 12 EPAP setting per my first sleep study that order by my family doctor. My sleep doctor ordered a new sleep study since he said my previous one result is not clear, it says I have both central and obstructive, he thinks the cpap causes the central. He said based on my jaw pb and 1 hour appo, he thinks I only have the obstructive apnea. So I am waiting for the second sleep study.

CPAP therapy pressures do cause centrals in a small percentage of obstructive sleep apnea people who didn't have centrals in the diagnostic sleep study. Sometimes there's not enough of them for there to be a real problem and sometime they go away on their own but sometimes a different machine is needed when there's a lot of centrals happening.

Get that second sleep study to see for sure if the centrals you are having are really a problem or not and then you will have a better idea what machine is needed.

With your pressure needs...I would for sure want to get a bilevel pressure machine even if it isn't the one that is needed to treat centrals (if you don't have enough centrals to worry about).
