Hey....the AHI was cut in half. Still ugly but there are short periods of time where not much is going on.
Only one break in therapy seen during the night.
How many times did you wake during the night?
How do you feel about the pressure used last night?
The goal was to improve sleep quality and not necessarily bring the AHI to below 5 at this point. Though I am pleasantly surprised to see that the AHI reduced. I sort of expected it to get more ugly since we limited the max pressure from where it was wanting to go.

It was good, that one break was me getting up to use the bathroom.... was the only wake up I remember / pressure was fine
I did get an appt with my family Dr who thru the info I shared that ive learned here/ sleepyhead info etc - prescribed me a sleep study right away! So that will be helpful as the last one was three years ago. Here is that Orig Sleep Report -

Turn off the mask pressure graph in SleepyHead so that the leak graph will move into view.
You don't need to show both mask pressure and the regular pressure graphs.
The leak graph is more important in this situation anyway.
To turn off unneeded graphs you can use the little box at the bottom right of the graphs...see the green...click on the box to expand it and then click on the mask pressure graph thing to turn it from green to red...red hides the unneeded graphs.
The settings don't quite match up to the settings on the PR S1 machine. Your EPR is at 3 and the most we could get on the PR S1 apap was 2...and this is not unexpected with the lower pressure settings we are using to get you acclimated to higher pressures. Also the ResMed algorithm is a bit different but that only affects the obstructive events. Both machines will ignore the centrals as they are supposed to ignore them.
Plus the leaks appear to have gone significantly into large leak territory. Some blank areas and some green UAs (Unknown Apneas) means the machine couldn't identify the category to put the event in because the leaks affected its sensing ability.
The AHI is predominantly central and really ugly. Unfortunately that is also not unexpected.
For tonight if you want to continue with the Resmed....try 12 minimum with max of 16 with EPR of 3.
Fix the leaks...while I will let a little large leak slide 30% of the time in large leak I can't let slide. That's too much time in large leak.
At some point I would like to see what happens with EPR reduced to 2 and then 1 but don't try it until you get used to the higher minimums.
It's good you got the new sleep study at least coming up fairly soon....you definitely really need it.
I think the pain medication is the primary culprit for the centrals.
I predict that you will be needing a different machine. One that will be able to deal with the centrals as well as the obstructive stuff.

Keep AutoSet (APAP) mode unless you want to try using about 16 cm pressure all night long. And that may not be enough.
I know you need more pressure for the obstructive events...we are ignoring the centrals for the time being since we can't do anything about them anyway except hope that they reduce if the Obstructive stuff reduces (and the chance of that happening is relatively slim)....so my goal right now is simply get you accustomed to the pressures so that the pressure changes to higher don't wake you up so much.

It's also pretty cool this machine goes up my .2 increments. Hopefully we can get some headway n then with the new sleep study and new Dr can get the proper machine if it's needed for the centrals as you have mentioned. Hope my insurance wouldn't cover that if not I'll figure that out then.
Also the girls that work the front desk at the drs office think that the sleep study may or may not have changed were it might be done at my house ? Vs at the sleep center. Will see soon when that office calls to schedule an appointment, so glad that's going to be sooner rather then later even though it is weird being hooked up to all those wires n glued to my head lol.
Are centrals related to my brain saying when to breath etc? I'll have time to do some reading at work tonight I expect a real slow night so I'll look thru for detailed info so I understand what all these things mean n terms I am unsure of etc...

I hope they do an in lab sleep study and not a home study. The home study may not do a good job of flagging centrals (depends on what level of home study gets done) so in your case I think an in lab study is definitely needed. In your situation with your history and meds.....I think an in lab study would definitely be the better choice.
Ramp can be use for a short period of time (like maybe 20 minutes) if you find the suggested pressures simply to much of a fight.
It's sort of trade off...getting to sleep vs sub optimal pressures during ramp. In your situation getting to sleep is of primary importance IMHO. We already know that the pressures being used aren't optimal but if you have too many apnea events while on ramp then the apnea events also could potentially wake you up.
So sorta damned if you do and damned if you don't type of thing.
Usually with EPR at 2 or 3 the need for ramp is lessened but you are getting up there in pressure needs so sometimes EPR isn't enough. If you decide to use ramp...start it out at around 7 cm and not the 4 cm normal starting point and try to keep it just for the time frame it may take you to get to sleep.

Here we go with last night, such a noob just noticed I could take other screenshots and well i have not yet read the Pugsy Pointers Thread so i'll post them all.....
Seemed like an ok night, took an ambian right before bed, woke up once to use restroom.... Am i too shut the machine off and back on when i get up like that or just take tube off n let it blow?
I will know the sleep study appt today!

Not sure if last night was "better" or not. It seems to be a "truckload" of things going on - I hope its slow enough at work ( A Beachfront smaller hotel in Delaware, we close for the season in a month).... that I can read that Pugsy's Pointers on Sleepyhead and other useful material - If anyone has any links to share please by all means I am game for reading and learning all I can

Bla - Sleep study is going to be Nov 14th - could be sooner she said, guess the insurance process takes up to 18 days if its before that then they said they might be able to make it sooner.
