Need help with recent ASV results with AirCurve 10

I had 5 central apneas and 18 central hypopneas. There were 239 PLMS with an arousal index of 10.3. The study said the Periodic limb movements were too numerous to count.The swallow arousal index was 7 per hour. I sleep with a night guard but bruxism was documented. I wake up exhausted every morning with yellow happy faces on my machine. I am using an AirFit N10 mask which is leaking with pressure of 4. Any suggestions on what to do about the PLMS? The doc said I must have GERD because I swallow so much and had me buy under mattress bed wedges which I could not get used to. Any suggestions on swallow arousals? Any suggestions on masks? I am a mouth breather but my doc wants me to wear chin straps and doesn't like the full face mask.

If you are getting leaks at pressure if 4, then its a really bad fit to your anatomy or real adjusted badly.
Or, your jaw is dropping and letting the air out that way.

Mouth breathers do better with full face masks or even oral masks.
If you don't have one, get a prescription for a cpap mask of patient's choice and find something that works for you, the doc is not the one wearing it!!!
I a think your PLMS is less severe than mine was and my doc prescribed Pramipexole to treat it. First, however, have your iron levels checked because low ferritin can be a cause and can be corrected with appropriate iron supplementation.

When you a get an appropriate mask, drop the chin strap unless the mask is some variant of a nasal mask. Do get a soft cervical collar to help with jaw drop.

In your next post, please list your pressure settings so there can be some informed comments about what is happening.
Also, get a copy of all of your sleep studies, especially the titration study - that's where a lot of really good information can be found.

Thanks so much. Maybe I will ask DME to try full face mask. I had a problem with leaks with the one I had and doc pulled me off. The Doc and DME are tired of dealing with me and only tell me not to fight the mask!! I have titration study. I believe the pressure settings are EPAP support 4 cm 1 MIN 3 1 MAX 8cm H2O.

I ware a full face mask, Simplus by F&P and when adjusted correctly I forget it's there. My issues is getting the machine settings correctly so I don't fight the CPAP. It would seem like it's hard to tell exactly what is being fought. Some plots from sleepyhead would be helpful

That's too bad for them. CPAP takes some adjustment and time. Took me a solid 3 months, replacement machine and RX before I started to get comfortable with it. Never ever saw a sleep doctor though, just the sleep center and DME, my PCP is the one that got me a better RX to get a more normal machine.

If your mask is leaking a great deal, as it appears to be, then you probably can get some better results by adjusting it. Tight, but not too tight. Too loose and it leaks. Too tight and it leaks. That "Goldilocks" zone can be frustrating to find.

Worst case, you might need to really push for a full face mask. I personally use the Quattro FX. But it's not necessarily the best for everyone. Why a full face mask? The ASV therapy includes the chance for some pretty wild pressure swings - as it helps sustain your respiration. If your doctor says you don't need it, ask him if a patient with non invasive ventilation would have just a nasal mask.

Another suggestion to help you acclimate to your new ASV unit is to wear it while you read / watch TV / listen to an audio book. Wear it so it becomes natural and normal to wear it. It takes time to adjust to ASV therapy. The more time you wear it - even when awake - the better your use while asleep will be.
