Late Sleep Cycle Cluster Events: Strategies?
I love this board, as its the first place i learned
1. Most people here know more about apnea than their doctors do
2. Its not the "average AHI" but rather how you FEEL next day and preventing major clusters
In my case , I can take a nap during the day one hour without machine (NOT RECOMMENDED! but i "can") and have few problems. My sleepyhead data always shows the first hour of sleeping is event free more or less.
As I approach maybe the 4-6 hour point of a 7 hour sleep sequence my "lower than 1 AHI" numbers can skyrocket into something like 6, and my periodic breathing can do the same.
So Im thinking: Alarm clock before it happens? Increase the pressure after 4 hours sleep? Or simply dont sleep more than 4 hours at a shot, split it up?
Have no idea, anyone help? Because if i go to cleveland clinic sleep center my 2.0 "average" AHI looks great to them. They generally just dont care about "EVENTS".

That sounds like a crybaby to me, but I will play along.skyrocket into something like 6
How long are those events? How is your energy level the next day?

Resmed does not use the word VSNORE but only SNORE. S9 Autoset knows who to eliminate it almost completely.

By "skyrocket" to 6 I assume you mean the average for the night goes from 1 after 6 hours of sleep and moves up to 6 after 7 hours. If that's what you mean then your AHI during the last hour is close to 40 which is quite bad. Not sure what to suggest but I first want to confirm that that's what you meant. Otherwise, I agree with Chicago.

Beyond that, what is the breakdown of "events" in the period that they're higher........more apneas or hypopneas?
Could be increased REM at that time........but, could it be positional? (only you know what your sleep position could be)
You should be able to see a pattern in your reports. You need to do a little sleuthing.

I have the same issues. My last hour is almost always in the 10-15 range. Mostly Centrals, with Periodics as well. I attribute them to my waking up. Often I will wake up for a minute or two about an hour before getting up, and if I do, I turn the machune off for a few seconds and then back on. That way I can turn the last hour off in Sleepyhead and get a better idea of my true AHI.

Seems to me your true AHI for the night should contain the last hour, bad or good. Why screen it out?

I like to see what it would be without it. I have a lot of centrals as I am waking up. It has nothing to do with OSA, for which I am being treated. Actually, my AHI really doesn't matter a bit. What matters to me is that I don't fall asleep during important conversations.

If you are having centrals during REM time I would not use an alarm clock to interfere with REM as it is very important to your well being to have REM. I would ask your sleep specialist what to do if your AHI on a daily basis is >5.

I don't use an alarm clock at all. I don't do anything to intentionally interfere with sleep! However, when I wake up I usually turn the thing off and back on to document when I woke up.
As an example, last night I slept from midnight until 605 with an AHI of 5.1.
Between 605 and 716 my AHI was 21.2 because I was waking up. So it works out to a 7.22 for the night. I just like to be able to break it down. My OHA for the night was .59 and for the last session, was 0.0.

How many As and how many Hs? How long were the As?Between 605 and 716 my AHI was 21.2 because I was waking up.

I had the same issue when I was in REM I was having clusters of OA's. The 2 to 3 hour period just looked terrible on the sleep reports. I was feeling OK, but thought I could fine tune it a bit. I ended up bumping up my EPAP setting up from 17 to 19.
The amount of episodes dropped and my AHI is now mostly under 1 where previously it was alway 2 to 3.
