That sleep doctor/author in Albuquerque has written that false negatives are a common problem for sleep labs. Looks like you found one.Physician wrote:Last night's values:

Exactly. I had 2 false negatives until I flew out to see Krakow to get the real story (Complex Sleep Apnea + UARS!)

She will get the repeat and appropriate studies within two weeks. In the meantime she is feeling better on CPAP.

What a horrible condition. Not everyone is so lucky to get the kind of treatment you provided. Good work Physician.

Did Krakow use an esophageal probe?justinjustin wrote:That sleep doctor/author in Albuquerque has written that false negatives are a common problem for sleep labs. Looks like you found one.

Also, how does one objectively define a false negative in re: NPSG? Do you get the diagnosis (in this case, UARS) stuck in your head, keep having NPSG till one comes out "positive", then dismiss all the others as "negative"?

Re: Here's another question.
Becasue it is NOT required:Physician wrote:Another question: If an esophageal probe is required for this diagnosis, then why didn't her sleep study and all sleep studies include this probe?
With respect to scoring a RERA, use of esophageal pressure is the preferred method of assessing change in respiratory effort, although nasal pressure and inductance plethysmography can be used.

Re: New UARS patient? Standard Sleep Study NORMAL.
Was hopin' you'd hit this thread, Sludge. Feel free to correct or refine or rebut any of my earlier nonsense in it.
Re: New UARS patient? Standard Sleep Study NORMAL.
Sludge,
A really stupid question.
In one thread, you asked if Krakow had used an ES Probe with the inference being this is the standard treatment for diagnosing UARS. And another thread, you said it is not needed to diagnose the condition which seems to agree with Dr. Park by the way. Can you clarify your position which seems contradictory even though I am sure I am missing something obvious?
The other issue is regarding labs not diagnosing UARS. I was told by one sleep doctor I saw that labs in my area do not test for it and I live in a big area. So if JustinJustin has encountered a similar situation and I don't obviously know that for sure, why would you say he was looking for a diagnosis? And by the way, just speaking generally, I agree that people definitely do that so in that sense, I appreciate the point you are trying to make.
Not looking to be argumentative by the way but just trying to deepen my understanding of what you are saying since you are a sleep professional.
Thanks!

I simply "asked" that.
I certainly must apologize if I appeared to "infer" that.

No apologies necessary. Like I said, I am just trying to understand your point of view and I don't mean any offense with the questions I ask. That is why I am glad you corrected me.Sludge wrote:But I did not "infer" that.49er wrote:Sludge,

By the way, some sites do seem to infer and I think I am reading that accurately although maybe not , that using the ES Probe is the best way to diagnose UARS. So that could of been another reason for my misinterpreting your post. So thank you for your patience in putting up with my questions.
Ok, so how should UARS be diagnosed in your opinion?

Sludge, I still would welcome your opinion but if you don't want to provide one, that is fine.
