Medical tag for Cpap bag for Airline?

Does anyone have a tag I can download and print out and laminate for my cpap bag for the airline? or do you guys know of a place that I can get a tag that dosnt have advertising plastered all over it?? thanks so much.

Thanks for asking... i got a free one when I ordered a mask online; and there appears to be no standard generic form. But I did run across this: http://www.medids.com/free-id.php and you might like it. It has minimal 'branding', and you personalize it with your info.

Doubt it's necessary.

Myself, I travel with the pertinent portion of the CFR regarding my right to transport an electronic assistive breathing device pursuant to the Air Carrier Access Act tucked into my bag. I would use the phrase "electronic assistive breathing device" on any tag because it tracks the bureaucratese of the regs.

TSA materials can be handy to have on as well, e.g. you can make them change gloves before they start pawing at your machine.

I once had to show my PRESCRIPTION to the confused flight attendant on a domestic flight. I think she thought I had a ventilator.
