New ASV user: Sleepy & tired as before

Post by MegaSleeper » Sat Jul 08, 2023 4:08 am

I need help to set up the right pressure as I don't feel that good yet. I am sleeping long hours like 12+ and find it hard to wake up, heavy head(like head is squeezed), tired eyes etc . I do have bad sleep hygiene. I am trying to fix it but that might not be 100% possible as I sometimes I have to work at night.

I have attached 2 reports with 2 pressure adjustments. With Min EPAP 7.0 Max IPAP 30.0 PS 5.0-15.0, I had aerophagia. with setup Min EPAP 4.0 Max IPAP 20.0 PS 4.0-10.0 didnt have aerophagia.
I use soft collar & mouth tape.

Sleep study showed CA events. as the OSCAR reports(resmed machines?) doesnt show CA events, how do I know its being treated?

You'll get it.
But the people who understand ASV's have to be up and about, and connecting to the forum.
They are worth waiting for.

Sleeping 12+ hours is probably just as bad as not getting enough sleep (if not worse). I understand that shift work makes it very difficult, but try to get your sleep hygiene sorted and target 8 hours of sleep.

The results aren't hugely different, though pressures are running a bit higher on the Thursday night when you had the aerophagia. I think the Friday night result looks acceptable. But you know that our sleep changes from one night to the next, and we look for trends, not one-off situations. But based on what you've shown us, the Friday (lower) settings look OK.
Sleep study showed CA events. as the OSCAR reports(resmed machines?) doesnt show CA events, how do I know its being treated?
If they weren't being treated, they'd show up in your charts as unclassified apnea. The fact they aren't showing up just demonstrates that the machine is doing its job.

I can't remember. :)

Thanks Respirator99 for taking the time to look at my post.

I tried 2 other settings in the last few days. reports are as below. which is the best result according to you?

Min EPAP 4.0 Max IPAP 13.6 PS 2.0-7.6

Min EPAP 5.0 Max IPAP 20.0 PS 5.0-10.0 (mild aerophagia)

I will continue few nights based on the suggestion and report back.

I spent few nights with these settings. I feel no difference using or not using ASV.

Megasleeper, have you ever been diagnosed with central or complex apnea, or are you self-treating for something else using ASV?

Whether you feel the difference with ASV or not, your treatment appears very effective with EPAP 4-10 and PS 4-10. Your overall pressure is lower and the ASV appears to be less active when PS min is 2.0 rather than 4.0. I think you might do better with comfort with PS min at 2-3 rather than 4, but I'm not the one experiencing the therapy. Give PS min 3.0 a try and give the therapy some time. At this point you are titrating comfort and sleep integrity rather than AHI.
