I have private insurance, not Medicare, Medicaid. I have in the past been reembursed for purchases from cpap.com at out-of-network rates. As the page you provided states:

Which matches my examples. I stated up front, I was providing an example that I had recently used.The pre-populated form may be used to seek reimbursement from your insurance company. CPAP.com is out of network for private insurance companies.

But you are NOT the vendor. You are submitting an invoice to your insurance provider "from a vendor which is out-of-network". Which is why I responded that is not how that works.If YOU submit an invoice to your insurance provider, then YOU are a "vendor". If you're also an insured member, then it should be logical that YOU are in-network as well.
