New specialist and re evaluation - help please!
New specialist and re evaluation - help please!
Had a myocardial infarction and stent placed 5 years ago at the age of 53. 157 lbs BMI 24. Retired air traffic specialist. 30 years of rotating shift work. Cardiologist referred me to a sleep specialist when I asked for zolpidem. Sleep study showed mild apnea - AHI 8.0. Took the zolpidem nightly and complied with CPAP until this past spring when I stopped the zolpidem and terminated the CPAP. I have felt fine since. Maybe not totally refreshed every morning, but no real sleep problem like years ago.
Primary care physician would not sign DMV form and referred me to different (and better) specialist. The new specialist is not requiring me to use the CPAP. But agreed that my problem appears to be complex apnea and suggested a new titration study for an ASV machine. I agreed and will undergo the study on Dec. 2.
My question concerns my inhalation and exhalation times from Sleepyhead. They appear to be reversed from normal. Would a dear forum member please take a peek at my data and comment?
Thank you in advance for your comments and advice. The forum is a treasure trove of information!

Your link goes to a download screen which I am reluctant to use... please do a link to the screen where the actual data is, if necessary use Imgur.com to input your stuff and then link us.

People aren't going to want to download your entire SD card and run the software to see the reports...and your SD card contents is what you have linked to. I don't have that much time available right now and would prefer you used the software available (see my signature links if you don't know about the software) and just posted a couple of typical nights detailed reports.
Normally we do screen shots of the daily detailed software reports.
Explained here along with some examples.

well, I see what you're talking about, often, on average, long slow inhales and very quick exhales, not every night.Golddust wrote:My question concerns my inhalation and exhalation times from Sleepyhead. i:3.34 e:1.10 another night, I:3.84 e:1.2, i:3.9 e:1.14 (median numbers)
I have no idea what, if anything, is the significance of that, though.
I also episodes of classic cheyne stokes respiration, some days a lot of it. (like nov 7) so investigating an ASV would be a good idea.

Also a lot of large leaks ... did you change masks last week ?
If you're not part of the solution you're just scumming up the bottom of the beaker!

Re: New specialist and re evaluation - help please!
These are your stats:
It looks from here like you're getting a handle on large leaks. For the first bit, it looks like you've got lower numbers, but that was during a period when your leaks weren't as well under control. Now that your leaks are under control, we're seeing more valuable data.
You've gone from AHI 26.33:
To AHI 11.92:
Then to last night's AHI of 5.73:
So trending toward lower AHI appears to be on track as your body gets used to the machine.
Your CSR seems to have reduced over time. There's only a little from last night. You're seeing a lot of Clear Airway Apneas (CA's), but you're also seeing some Obstructives (OA's). Only a very few people have nothing but CA's. You might be one of them, or you might be getting centrals from your pressure being to high. Then there are the huge number of hypopneas, which sometimes mean upping the pressure might help. It looks like things are getting better all around though. It's only been a month. I wonder if just leaving it at this pressure might be the best course for now to see how things settle.
Unless you're perfectly comfortable having your data out there, it couldn't hurt to remove the link to the zip file you created. Just a thought.
I hope this helps.

I am kinda new to this apnea thing, so I am not an expert,.... your data looks like mine did when I had pressure to high .... and if those were my data ... I would set it to auto of 6/9 for a few days and see what happens.

Re: New specialist and re evaluation - help please!
I think you are hand-picking data to show the decrease in AHI. Leak rate or not, the data is all over the map. There is no clear trend. So trending toward lower AHI appears to be on track as your body gets used to the machine.
I think OP is an excellent candidate for ASV.
Re: New specialist and re evaluation - help please!
agreed, dreamdiver's analysis is only valid if you look at the last week or so's data, but if you go back the full month, or even longer, things are worse, not better.cnaumann wrote:I think you are hand-picking data to show the decrease in AHI.
