Best CPAP, APAP machine to purchase in 2017
If I was to purchase a new CPAP or APAP machine what would be the very best one to get? Cost isn't a factor.

It depends on your diagnosis.
But for straight APAP I like the Airsense 10 Autoset for her.
If you require bilevel or ASV I am out of date. I am still wondering which one I should have selected 3.5 years ago. In 2018 I will need to start the quest again, for a new ASV I am hoping everything available today will be obsolete by then.

It has cpap mode, fixed bilevel mode, auto bilevel mode plus it can easily be configured to mimic APAP mode.
Not cheap but covers all potential bases except for what ASV or backup rate machines do for centrals and you don't need central treating unless you have a truckload of centrals.
FWIW I have owned both brands (Respironics and ResMed) in this bilevel category and while there are very minor pros and cons with either...overall I prefer the ResMed. I think it is a little quieter and the algorithm it uses in auto mode lets me use a little less pressure at the beginning of the night because it responds a little faster to obstructive sleep apnea events or warning signs.
The only thing the Respironics 760 auto bipap will do that the ResMed can't is vary the PS or pressure support in auto bilevel mode. I have yet to find a situation where having a variable PS is that critical of an issue with this type of machine. With ASV yes but not with this type of bilevel.

Does it collect data and if so, are you monitoring your therapy with software?
What is your diagnosis? Simple "garden variety" OSA or complex with Centrals and Obstructive?
Is your present equipment doing the job?
Does it provide you with data to monitor with software?
And, just remember......whatever you buy THIS year will be obsolete and out of date in a couple more years.
(but it will probably still work and deliver the same therapy it did when you bought it)

I have a Phillips Respironics Dream Station Auto CPAP. It’s my fourth machine since 1999; the others were all “bricks.” Here’s what I think of it; the good and the bad.
Advantages:
• Automatic means that it will raise the pressure as needed when it senses distress. I think I fell more refreshed than before.
• Good statistics are generated, so you can see how you slept, and make adjustments as needed. In addition, stats are good for your doctor to see how you are doing.
• Machine is very quiet.
• Heated hose is very good, never had rainout despite full setting on humidity.
• Automatic altitude adjustment (good for travel).
• Leak compensation.
• Auto/on and auto/off can be activated and deactivated separately.
• Puts out the prescribed pressure more precisely than my old machine (yes, I verified this).
Disadvantages:
• Machine is a bit larger than the major competing machine.
• Sometimes, there is a burning smell, especially after a potty break. I’m pretty sure it’s harmless.
• Frequently, the water does not last the whole night. I generally wake up at least once overnight, so I refill it at that time. This might not work for everybody.
• Some have complained about the brightness of the display when you turn it off and back on, such as for a potty break. I don’t find this a problem. In fact, I use the light to reattach my mask to the hose.
• The power cord has a proprietary signal, so you cannot use a different cord, even if the voltage, wattage, amperage, and fitting are all identical. This means that if you need a replacement cord (or a spare cord), you have to get the cord specific to Dream Stations. If you want to use a battery (such as for camping and electrical outages), you have to buy an $80 proprietary device to generate this signal.

The ResMed S9 for sure. The AS10 has the guts of the time-tested S9, but is noisier. Twoi DMEs have told me they consider the S9 more reliable.

Is there a machine with ASV that can work at relatively low pressure support ?
I am looking for setting with E (5-6) I (7-9) and ASV (PS) (0-2)
I do not need too much pressure to handle obstructive apnea. Higher pressure is inducing CA, UA, leaks and unfortunately terrible aerophagia and gerd.
