It doesnt seem much of a stretch to think that untreated severe OSA could cause brain lesions and any other neurological problem if untreated long term. Sorry to hear of your father. My dad is just like me----kicks out at "attackers" in his sleep. Much of this behavior is gone now that im compliant....

I was in carolinas med center charlotte 2 days and kudos because they had a great respiratory guy who'd come in and make sure you were set up. The problem was the bed. Hard to sleep sideways there. I wonder if youre preventing major event sequences in late stage sleep.
I know for me, especially if i go over 7/8 hours in a day, that the last hour can be brutal with the number of events.

Ive never heard that side sleeping was better than stomach sleeping before. For one thing its the position of your head that matters it would seem, not your stomach. So your head is on its side, not face down.

Overwhelmingly my highest event period is the last of the night.
The first 90 minutes of sleep is always almost event free.
So if you'd said the first hour was no mask, and the last 4 was mask, i might say that theres
a chance you wouldnt be doing significant damage.

I agree with your postulation that eventually pap will be considered an antiquated treatment. But it WORKS, which is a breakthrough. For millenia people just died of "strokes" instead of having some available treatment. So definitely pursue the septum surgery, but make sure you choose a sleep surgeo...
