From my experience at the 50% mark you will be better off purchasing online without insurance. Of course how much better depends on the machine that you choose, and the machine you would have accepted from a brick and mortar DME. Many things will whistle when they are not seated tightly and vibrate. This kind of attitude is typical of doctors and hospitals in general. About 10 years ago I was told by a hospital that I could not have a copy of the xray of my leg. Of course I knew better and refused to leave without it. Once they understood that I KNEW that it was my right, they caved in a hurry....
