My average AHI is exactly the same where I live (at 200 ft above sea level) and over a 19 day stay at 5500 ft. There are two issues here. 1) The XPAP adjustment to different elevations. As I understand it, most newer XPAPs (including the Resmed S9 line) do adjust to elevations atmospheric pressures. Medicare paid for both my original ClimateLine (heated tubing) hose and for a replacement, so I doubt that you will have any problem getting them to pay for yours.

I don't wake up with "plugged ears," but on some occasions when my APAP treatment pressure has been unusually high during the night I wake up a slight earache in one or both ears.

I don't use EPR either. After extensive experimentation it was clear that my AHI averages lower and I sleep better with EPR off regardless of other settings.

I have both Respironics and Halo Chin Straps. The Halo Chin Strap is easier to put on and take off, and is considerably more comfortable to wear. I have only tried those two, but the Halo Chin Strap works well for me and is far better than the Respironics in my opinion.
