Rest in Peace

To all of our father's friends on this blog: Our father, John Dismuke, passed away last week on December 4, 2008 from a heart attack. Thank you to everyone who supported and made friends with him near and far. Although we are deeply saddened by our loss, we are at peace with his passing and surround...

Re: Need some help on settings please

-SWS you left off the part where EPAP will never increase using those settings for Hypopnea, IPAP will simply run up to the max trying to kill hypopnea with no help what so ever seen from EPAP, all that does is result in a much higher PS than what may be optimally needed. By contrast If Hypopnea was...

Re: Received a new machine

Bill had a bipap plus. He does need to carefully check the new script. If the doc just wrote apap or auto, then the DME can bill the insurance for a new machine as this is a change in treatment. If the new script indicated bilevel need, then the DME can balk and say the existing machine (Bipap plus...

Re: the rest of my sleep study test (cont)

I am back from the doc, and I returned the CPAP. I did not get to speak with the doc, and for me to get a VPAP would cost me $1,700.00 out of pocket. My doc will not make the VPAP medically needed. Kaiser is not willing to here what I have to say. I was look for a new doc and get back on the hose a...

Re: Adderall, melatonin and other sleep disrupters & fixes

Geeze what a basket case! but you better stick with the Adderall, it treats severe depression, symptoms of narcolepsy and idiopathic CNS Hypersomnia. So if you have any of those you better stick with it. This is a ton of posts here on Melatonin, you just have to search for them. But I take between 3...

Re: Surgery as an option?

I am a new CPAP user (1 week in and going well) but I keep having this circular debate with my wife. Wife: So, you will have to use this mask the rest of your life? Me: Yep. Wife: Why don't you get a surgery - my brother and your cousin both did? Me: I don't like the idea of someone cutting my thro...

Re: My first night on ASV

James, everything besides hours 1-to-2 look clinically acceptable. Do you know if you were side sleeping, stomach sleeping, or supine sleeping during hours 1-to-2? Supposedly some CSDB patients have more problems with central dysregulation when they side sleep. Wonder if there's a pattern of extrem...

Re: New CPAP user can't stop sneezing. Help!

I have just spent two nights on a loaner machine (before getting my own equipment). I have been sneezing and sniffling all day. Is this normal? Many thanks for any help you can give. I would call and report it to the DME, could be something they sprayed on the machine to clean it, take the filter o...

Re: My first night on ASV

I thought some might find this useful in respect to the conversation: http://james.istop.com/apnea/reports/cycle.png Two points of interest regarding BPM and IT ratio calculations: 1) The overall BPM is determined by that recurring period from the rise-time/trigger line to that same point at the be...

Re: Sandman Auto - IFL and max P. on apnea

on the very last page of the Respironics Titration Guide they state: CENTRAL SLEEP APNEA (CSA) IS DEFINED AS: 1. An apnea hypopnea index (AHI) greater than 5; AND 2. Central apneas/hypopneas greater than 50% of the total apneas/hypopneas; AND 3. Central apneas or hypopneas greater than or equal to 5...

Re: My first night on ASV

What IT= time did you end up using? Initially I tried 1.7 as you first suggested. I then tried 60/14.5/2= 2.06 which was your second suggestion. Finally I tried 60/12/2= 2.5 which was my calculation Okay, that is good to know, so a tiny change of IT time makes a big difference. The patient triggere...

Re: My first night on ASV

http://james.istop.com/apnea/reports/AsvNov28.pdf I honestly don't think the crux of the problem is that IPAPmax (the ceiling) is not set high enough. Here's another autoSV spin, looking at the above graph. Specifically I'm looking at hours 3.5 to 4.75. That could very well be James hitting heavier...
